Chiropractic Care During Pregnancy: A Trimester-by-Trimester Guide

While carrying a child and bringing them into the world may be a beautiful experience for parents, it is not always rainbows and butterflies. Pregnancy can be hard on you as an expectant mother. As your belly grows, your posture shifts, your center of gravity moves forward, and the muscles and joints holding everything together start to feel it.

Most pregnant women push through it, assuming seeing a chiropractor during pregnancy isn’t an option.

But chiropractic care is safe when performed by a trained prenatal chiropractor, even when done throughout all three trimesters. It can meaningfully reduce the discomfort that comes with a changing body that is busy creating new life.

We break down what chiropractic care looks like at each stage of pregnancy, helping you know what to expect, what’s safe, and how it can support your body.

Why Pregnant Women Should See a Chiropractor

Pregnancy shifts your entire center of gravity. Your lumbar curve deepens, your pelvis tilts, and the relaxin hormone loosens your ligaments, all at once. In fact, 50 to 80% of women report back pain at some point during pregnancy.

Moreover, many women don’t realize that pregnancy can cause physical changes beyond back pain. These changes impact movement, nerve function, and the body’s adaptability to each trimester’s demands. If not addressed, these issues can worsen, leading to increased discomfort later.

The American Pregnancy Association supports chiropractic care throughout pregnancy because it helps maintain spinal alignment, improves pelvic balance, and reduces the nerve stress that contributes to pain and discomfort.

For expecting mothers, the benefits of seeing a pregnancy chiropractor go beyond back pain relief:

  • Better overall pregnancy health through improved spinal and pelvic function
  • Reduced nausea symptoms through improved nerve communication
  • Relief from back, neck, and joint pain without medication
  • Shorter labor and delivery time
  • Lower likelihood of cesarean delivery

Seeing a chiropractor during pregnancy is one of the few drug-free options available for expectant mothers when medication choices are limited. Getting care early means your body is better supported through every stage ahead.

Chiropractic Care by Trimester: What to Expect at Each Stage

Chiropractic care during pregnancy looks different at different points in your journey. Each trimester brings its own physical demands, so the approach should adjust accordingly.

First Trimester: Building the Foundation

Many pregnant people wait, believing the first trimester is too risky for chiropractic care, but that’s not true. Effectiveness of Manual Treatment on Pregnancy Symptoms, a study published by the National Institutes of Health, found that not only is chiropractic care safe during the first trimester, but effective in treating symptoms such as nausea and neck pain as well. It’s also a very ideal time to start, as your body is still in the early stages of change.

What’s happening in your body during this stage sets the tone for the rest of the pregnancy. Hormonal changes loosen pelvic ligaments, adjust your posture, and make it easier to address any spine or hip issues.

First-trimester chiropractic care typically focuses on:

  • Spinal alignment for nerve function: Helps the nervous system, and many patients notice that it reduces the severity of nausea.
  • Pelvic balance: Getting your pelvis properly aligned early makes it easier for your body to adapt as your baby grows.
  • Pre-existing condition management: Old injuries, back issues, and muscle imbalances are easier to address before third-trimester weight loads arrive.

During the first trimester, prenatal chiropractors use gentle, low-force adjustments. No abdominal pressure is applied, and positioning is modified from the first visit to keep you and your baby comfortable throughout. The earlier you start the treatment, the more comfortable your pregnancy will be.

Second Trimester: Adapting to Your Changing Body

The second trimester is often called the honeymoon phase. Energy is up, nausea has eased, and movement still feels manageable. But this is also when sciatica, round ligament pain, and postural strain start to set in.

As the baby gets heavier, the lower back carries more weight, and the pelvis leans forward more. This puts pressure on the sciatic nerve and stretches the round ligaments. That’s why pain and discomfort often increase during this stage.

Here’s what an ideal chiropractic care looks like during this stage:

  • Pelvic alignment: Accommodating your baby’s growth without adding unnecessary stress to your joints and surrounding muscles
  • Sciatica and round ligament pain: Relieving the nerve pressure and ligament tension that peak during this stage
  • Posture and mobility: Keeping you moving comfortably as your center of gravity continues to shift

Prenatal chiropractic adjustment at this stage uses pregnancy pillows and specialized drop tables with a contoured belly cutout, so you can lie face down safely. No abdominal pressure is involved, and the adjustment is as comfortable as it is effective. Many patients also pair visits with prenatal massage therapy for added muscle relief between sessions.

Third Trimester: Preparing for Delivery

The third trimester is when the body works hardest. The baby is heaviest, the pelvis is under maximum strain, and fetal positioning starts to clinically matter.

As delivery nears, the body produces more relaxin, which loosens the joints and ligaments in the pelvis. This, along with the baby’s size and position, often leads to more discomfort in the lower back, hips, and sacroiliac joints.

In the third trimester, care priorities shift to:

  • Optimal pelvic alignment:  A balanced pelvis gives your baby the most room to move into the right position for delivery
  • Reducing in-utero constraint: When the pelvis is out of balance, it can restrict your baby’s ability to move and reposition freely
  • Lower back and hip pain management: Regular adjustments keep your lumbar spine and hips functional and pain-free as weight increases.

By the third trimester, your chiropractor prioritizes comfort and safety at every visit. They use side-lying positioning and specialized support cushions to keep you comfortable throughout the adjustment. Techniques are gentle and adapted to your size and how your body is responding week to week.

In some cases, this stage is when the Webster Technique comes into play during pregnancy. It’s a specific adjustment that addresses subluxation and SI joint dysfunction in the pelvis and sacrum. When these joints are restricted or misaligned, it affects how the entire pelvic region moves and functions. Correcting that restores better movement, reduces discomfort, and creates a healthier environment for your baby’s positioning heading into delivery.

What to Expect at Your First Prenatal Chiropractic Visit

If this is your first time visiting a prenatal chiropractor, do not worry. Every step is designed with your comfort and your baby’s safety in mind. Here is what to expect during your initial visit:

  1. History review: Your chiropractor will ask about your pregnancy stage, current symptoms, and any coordination with your OB-GYN.
  2. Physical assessment: The chiropractor looks at your posture, spinal alignment, and pelvic balance to understand what your body needs.
  3. Treatment plan: A plan is tailored to your trimester, symptoms, and health goals.
  4. Gentle adjustment: Pregnancy-safe positioning is used throughout. You won’t be placed face-down at any point.
  5. Home care tips: Your chiropractor sends you home with instructions on safe stretches and proper posture to support your progress between visits.

Just a reminder: Always consult your OB-GYN before beginning chiropractic care. They know your full health history and can help ensure it’s the right fit for where you are in your pregnancy.

Postpartum Chiropractic: Recovery After Delivery

pregnant-woman-having-a-chiropractic

Birth, vaginal or C-section, puts real strain on your pelvis and core. Postpartum chiropractic care helps you recover faster and function better through the early months of parenthood.

Postpartum chiropractic care addresses a specific set of recovery needs:

  • Pelvic realignment following vaginal or cesarean delivery
  • Neck and upper back tension from breastfeeding and carrying your newborn
  • Core rehabilitation, as your abdominal muscles begin to recover strength and function

Your body did something remarkable. Give it the recovery it deserves with postpartum chiropractic

Frequently Asked Questions

Yes. Low-force techniques are used throughout, with no pressure on the abdomen. Check with your OB-GYN before starting, as you would with any new treatment during pregnancy.

The Webster Technique is a specific chiropractic analysis and adjustment focused on the pelvis and sacrum. It helps relieve joint tension and restore normal function in that region, allowing the pelvis to move as it should. Many prenatal chiropractors use it in the third trimester to support better fetal positioning ahead of delivery.

It depends on your symptoms and how you respond. Weekly visits work well for some; every two to four weeks works for others. Your chiropractor will give you a realistic recommendation after your first assessment.

The Webster Technique improves pelvic alignment, allowing the baby to move naturally. It’s a safe, conservative option to try before more invasive approaches.

Stiffness often occurs due to inflammation, scar tissue formation, or immobilization during healing, which can limit the range of motion and require targeted stretching and strengthening to restore function.

Yes, once your OB-GYN clears you, usually around six weeks postpartum. Care at that stage focuses on pelvic realignment, core recovery, and postural tension from newborn care.
